Hyperlipidemia
Hyperlipidemia (high cholesterol) is a critical, asymptomatic risk factor for cardiovascular disease. It involves elevated levels of lipids (fats) in the blood, primarily low-density lipoproteins (LDL) and triglycerides.
The Standard Lipid Panel
A standard fasting lipid panel measures four primary metrics in milligrams per deciliter (mg/dL). Modern clinical practice focuses intensely on LDL and Triglycerides as the primary drivers of atherosclerosis (plaque buildup).

Pathophysiology: Atherosclerosis
Atherosclerosis is a decades-long process. It begins when excess LDL particles penetrate the endothelium (the inner lining of the artery). The immune system responds by sending macrophages to consume the LDL, which eventually die and form a "foam cell." These cells accumulate, creating a fatty streak, which eventually calcifies into a hard plaque. If this plaque ruptures, a blood clot forms, blocking the artery completely—resulting in a heart attack or stroke.
Treatment Pathways
1. Dietary Interventions
- Saturated Fats: The most significant dietary driver of high LDL. Guidelines recommend limiting saturated fat to less than 6% of total daily calories.
- Soluble Fiber: Binds to cholesterol in the digestive tract, preventing absorption (e.g., oats, beans, psyllium husk).
- Triglyceride Reduction: Driven by cutting out refined sugars, alcohol, and simple carbohydrates.
2. Pharmacotherapy (Statins)
HMG-CoA reductase inhibitors (Statins) are the bedrock of lipid-lowering therapy. They work by blocking the enzyme the liver uses to produce cholesterol, forcing the liver to pull LDL out of the bloodstream. They are highly effective, heavily genericized, and possess anti-inflammatory properties that stabilize existing arterial plaques.
Common examples: Atorvastatin (Lipitor), Rosuvastatin (Crestor).
